  5. Use this form to order a multi-computer site license for SmartDir.
  6. This will allow the use of a registered version of SmartDir on
  7. the number of computers specified.

  9. LICENSED SITES RECEIVE...

  11. ò A Registration Code that will remove the report line limits and
  12.   closing "reminder" screen. All computers at the site can use this
  13.   same Registration Code.
  14. ò Priority technical support.

  16. ò Prompt notification of future updates.

  20. The following volume discounts apply to SmartDir site licenses.

  32. HOW TO ORDER

  34. CHECKS OR MONEY ORDERS

  36. 1) Print a copy of this form.
  37. 2) Fill in the SMARTDIR 3.0 SITE REGISTRATION FORM on page 2 of the
  38.    printed copy. All information will be treated as confidential and
  39.    will not be given to any third parties.
  40. 3) Make out a check or money order (in U.S. dollars drawn on a U.S. bank)
  41.    payable to Gavlock Consulting for the Total amount. Iowa residents
  42.    must add sales tax (5% plus any applicable Local Option Sales Tax that
  43.    may be in effect for your area). International postal money orders in
  44.    U.S. dollars will also be accepted.
  45. 4) Mail the completed form, along with your check or money order, to...

  51. After your registration form is received and your check has been processed
  52. a Registration Code and instructions on how to enter it will be sent to the
  53. designated Contact person. This will be sent either via an email note
  54. (to the specified Contact Email address) or via regular U.S. Mail (if no
  55. Contact Email address is provided).

  57. PURCHASE ORDERS
  58.  
  59. Purchase Orders (net 30 days) will be accepted for orders over $50.
  60. Submitted Purchase Order forms must include a purchase order number
  61. and a billing address. Please include a completed SMARTDIR 3.0 SITE
  62. REGISTRATION FORM, and mail it to the above address. You will be
  63. invoiced for payment by check or money order (in U.S. dollars drawn
  64. on a U.S. bank) or by International postal money orders in U.S. dollars.

  68. If you prefer to pay for your site license with a credit card, please
  69. send an email note to sales@gavlockconsulting.com. We will email you
  70. a form that can be filled in and faxed to our credit card registration
  71. service.
